Personnel Announcement

Mary “Peg” Boyle, JD MPH is the newest member of the HSPP group. Peg returns to the Seattle area as a Regulatory Analyst, having previously worked in the area as Human Subjects Analyst at the University of Washington and Fred Hutchinson Cancer Research Center. Peg comes to us from the Office of Protection of Human Subjects, Children’s National Medical Center, Washington D.C. Her extensive experience in Human Subjects Protection is an excellent addition to our great staff.

Laura Hill recently joined the HSPP team as Program Coordinator. Laura graduated from the University of Washington with a BA in Sociology: Law, Society and Social Policies. She has a strong background in administration and comes to HSPP with many years experience in healthcare. Laura will provide administrative support to the IRB and HSPP and will be a primary contact for IRB-related communications. We are delighted to have Laura join our group.

Revised HIPAA Authorization Form
As of February 2008 the new HIPAA Authorization Form is in effect for all new studies. Please use this Form when submitting a new study to the IRB. For studies currently open and enrolling participants, submit the new Form at the time of the next renewal.

Revision Highlights

  • Check boxes representing different types of information collected or used as part of the study must be selected as appropriate
  • The length of time researchers will keep participant information will now be represented by one of two statements, selected using check boxes.
  • The “permission” section provides clearer directions for collecting signatures from participants and parents. The researcher obtaining authorization must now also sign this section. Directions for filing of completed forms are updated and clarified.
  • The revised Form has been translated into seven target languages. Check boxes should minimize the need to obtain further translations but translations can be required in certain circumstance (see the Form for details). Directions to researchers are also in English on the translated Forms to ensure the Forms are completed accurately. Please use the English version as a reference.
